RF microneedling: the procedure, the downtime and what to realistically expect

How an RF microneedling session works at the studio, how long redness and crusts last, how many sessions you need – and who should not have the treatment.

Radiofrequency microneedling is the treatment we get the most questions about – and the most misconceptions. It is not a “needle massage”, not a laser and not a substitute for surgery. What it is: a controlled injury to the skin that makes the body produce new collagen. Here is how it works at our studio, without the gloss.

Before the treatment: the intake is not a formality

At the first appointment we take time for your skin and your history first: medication, existing conditions, previous treatments, a tendency to herpes or pigmentation. In Germany, RF microneedling may only be offered with certified expertise under the NiSV regulation – and part of that expertise is exactly this: recognising when a treatment should not take place.

We do not treat during pregnancy or breastfeeding, with active acne or herpes in the treatment area, metal implants in the area, a pacemaker, isotretinoin in the last six months, blood thinners without medical clearance, or a fresh tan. If any of these apply, we will say so – even if you have already booked.

Two weeks before: no peels, no retinol, no tanning beds. On the day, come without make-up.

The session: 60 to 90 minutes

  1. Cleansing and numbing. A numbing cream works for about 30 minutes under film. This half hour is part of the treatment – plan for it.
  2. Treatment. The handpiece places fine, insulated needles at a defined depth (usually 0.5 to 3 mm depending on the area) and delivers radiofrequency energy at the needle tip. The heat is generated where it is needed – in the dermis – while the skin surface is largely spared. The face takes about 20 to 30 minutes.
  3. How it feels. Warmth, tingling, briefly more intense on the forehead, jawline and nose. Most people describe it as tolerable, not pleasant.
  4. Calming. Cooling, regenerating care; optionally the exosome add-on, worked directly into the open skin.

Downtime, honestly counted

  • Day 0: redness and heat like a strong sunburn. No make-up, no sun, no sport.
  • Day 1–2: redness fades, the skin feels dry and tight. Fine, sandy crusts form at the needle points. Mineral make-up is possible from day 2.
  • Day 3–5: crusts come off on their own – please do not rub. Sport, sauna and swimming from day 3.
  • Day 5–7: the skin looks normal again, often a little fresher than before. That is not the result yet.

If you have an important date, schedule the treatment at least a week before. For most people a weekend is enough downtime.

The result comes later than you think

New collagen forms over weeks. Many see a first change – finer pores, more even texture – after four to six weeks. Tightening along the jawline or cheeks shows after three to six months, and after a series of three sessions four to six weeks apart. For acne scars, four to six sessions are more realistic.

What RF microneedling cannot do: “remove” deep wrinkles, lift sagging skin like a facelift or replace volume. Anyone expecting that will be disappointed – and we would rather say so in the consultation than after the third session.

After the series

A maintenance session once or twice a year keeps the result. In between: consistent sun protection, because UV light breaks down what the treatment has built up.
